Once you are in the office, you will be asked to fill out an application form. Therefore, it is important that you know some necessary information such as the name at birth, birthday, birthplace, your parents’ name as well as you mom’s maiden name, and the year of birth registration.
Now, the last thing to do is to fall in line, and wait for your application to be processed. This entails payment fee of 125 Php wait for its release and tada – you have it in one day. Its validity doesn’t expire. In cases of correction and change name, you may have to send docs supplementing the correction via mail to the NSO main office in Quezon City.

Get in online and have it delivered in 3-4 days. Payment may be made through credit card and other bank transactions. The cost of online application may be a little higher than how much you pay when visiting the office. The rate includes delivery price of the document. Roughly, the amount to be paid is about 300 Php.
